New Clients
Please arrive fifteen minutes prior to your first appointment to complete new client paperwork or you can download it below to print and bring it completed.
For adult clients click Here to download the new patient packet.
For adolescent clients aged 17 and under click Here to download the new patient packet. If you have a parenting plan or other custodial paperwork you MUST bring this to the first appointment so a copy can be placed on file, failure to do so could result in being rescheduled.
Please bring all current insurance cards with you to your appointment.
When you arrive please use the side entrance on the far right of the building and check in with the office manager at the front office. Should you have any questions prior to your appointment please feel free to contact us in office.
Insurances accepted and private pay rates are listed under each provider's name in the Meet Us Section
All Copays, Deductible amounts, and Private pay amounts are due at the time of service. We currently do not have any providers who accept Medicare at this time. If there is a question about how much you will owe please call the office and we will be happy to look up your insurance.
Our providers use a service called Jituzu to schedule and communicate with clients, this is also utilized should appointments need to be scheduled via telehealth.
You can download the app for Apple devices and Android devices through your prospective app store. Your provider will send you an invite code to join shortly before or after your first appointment.
